Nephew hit me with this, since I don't have access to the rifle I'm shooting in the dark. I was thinking maybe to lower is borderline, only showing up with the lower recoil impulse of the 5.56? .223 ammo issues, gas, too much buffer weight, carrier binding in upper, heck I don't have a real clue.

I had this problem switching from a .223 upper to a 9mm upper.
The .223 had no problem.
The 9mm worked perfectly when using my Slide Fire stock (Bump Method) but when shooting in semi auto the hammer would never lock back and ride the bolt (never causing double fires)
It drive me crazy. The 9mm bolt WAS moving all the way to the rear but the hammer never reset.
What I found was the hammer was hitting the sear.
When I had the trigger pulled it was not letting the sear engage. That is why it worked with the SlideFire stock because using this device makes your finger leave the trigger or ease up on it so it can lock up.
I had to take the Hammer to the bench grinder and grind off the rear "tail" until it no longer made contact with the sear.
Have him cycle the guns in each caliber while the trigger is pulled and with out pulling the trigger.
